I would like to Apply for Change of Curriculum to the Respiratory Therapy Program for Fall Semester at Bergen Community College

	I am admitted into a degree program (i.e. Liberal Arts, Professional Studies General, etc.) at Bergen Community College.
	Algebra Requirement I completed MAT 031 & 032 or MAT 035 with a grade of C or better or I was waived from taking Algebra due to Basic Skills Tests, SAT scores, or other college math courses.
	Chemistry Requirement I completed High School or College Chemistry with a lab (CHM 100) with a grade of C or better within the last ten years.
	Biology Requirement I completed High School or College Biology with a lab (preferably BIO 109) with a grade of C or better within the last ten years.
	Overall GPA requirement I have a grade point average of 2.0 or higher at Bergen. My Current Cumulative Grade Point Average is
