Enrollment Medical Requirement

Medical requirements for enrollment at
BERGEN COMMUNITY COLLEGE

The College requires all matriculated students, athletes, and enrollees in Wellness/Exercise Science courses, Nursing, and Health Professions students to submit a Health Record. The form must be completed and returned to the Health Services Office before registering for classes. This form has three sections:

Part A is for you to complete, Part B is the immunization section and Part C/page 2  is a medical exam which must be completed by a physician. A PDF version of the form can be downloaded or picked up in the Health Services Office in Room HS100.

New Jersey Department of Health immunization requirements:

Completion of this requirement is mandated by NJ Dept. of Health

1.  NJ College students registered with 12 credits or more, born after 1956, must submit PROOF of two Measles vaccines, one Mumps vaccine and one Rubella vaccine.

2. NJ College students registered with 12 credits or more, regardless of age, must complete Hepatitis B series within 9 months of attendance. 

Please read the form carefully and completely. Specific requirements are listed for the Nursing and Health Professions student. Complete all sections before returning to the Health Services Office. This information must be submitted before you can go to a clinical site.
