1.   a, b, and c  This item is testing “recall” or the ability to remember information.  You must know that them meaning of qid means four times a day.

2.   a,b, and d  This item is testing comprehension.  You must be able to understand the implications of previously aquired information.

3.   b,c, and d  This item is testing application.  In order to correctly answer this type of question you must be able to remember, comprehend and apply concepts to acutal situations.

4.   a,c, and d  This item is testing analysis.  When you answer this question, you must understand and examine the information presented, identify the relationships and arrive at a conclusion.

5.  a,b,. and d  The stem with negative polarity is concerned with what is false.  NCLEX does not focus on these questions.  You may see them on your tests in class.  First decide on which answers are true, you are looking for the false answer as the correct choice.

6.  b, c, and d  This question is testing the “assessment” component of the nursing process.  The assessment process focuses on the collection of data using all your senses.

7.  a,b, and c  This question is testing the “planning” component of the nursing process.  The nurse is the co-ordinator of planning patient care.

8.  b,c, and d  This question is testing the “implemtation” component of the nursing process.  This item tests your ability to recognize correct rationales for nursing procedures.

9.  a,c, and d This question is testing the “evaluation” component of the nursing proces..  The nurse must evaluate the effectivenenss of patient teaching.

10. a,b,  and c  This question is similiar to number 5.  Many students get these type of questions wrong because they do not carefully read the question.
